Following ACSM guidelines, medical clearance depends on a few factors: current activity level, any history of cardiovascular, metabolic, or renal disease, and the symptoms questionnaire completed during intake. Clients who are already exercising regularly and screen clear of relevant symptoms typically don’t need a physician’s note to begin. Clients with flagged symptoms, a relevant diagnosis, or who are starting from a sedentary baseline and aiming for higher-intensity training may be asked to get clearance first. The intake form determines which category applies before the first session.
